Case Management

Case Management Services shall consist of rehabilitation, recovery,  environmental support, and targeted case management activities which are considered essential to assist the person served in gaining access to necessary services; and in the provision of rehabilitative services intended for maximum reduction of symptoms of psychiatric illness to restore the client to the best possible functional level; and which are identified in the ISP of the person served.  Program goals include:

Reduce psychiatric hospitalization
Assist the person to function more effectively in the community
Assure that basic human needs are met.
Reduce impairment of mental illness.
